Coding by Accident
July 16, 2008
The Wall Street Journal health blog posted on the fact that there is an ICD-9 code for a spacecraft accident. While I do find that amusing, I am not sure people realize the myriad of crazy ICD codes related to accidents. I am not sure why this is such a common need for specific codes. There are many circumstances where I need an ICD code for something fairly common, but can’t find one. Why would this happen with some medical conditions, while having an incredible array of codes for differing types of accidents? My guess is for workman’s compensation.
Anyway, here is the list of codes associated with the word “accident” exported from our EMR. I have highlighted and commented throughout.
This is real. Honest.
Enjoy.

“Abnd” = Abandonment.
July 16th, 2008 at 8:25 pm
Dizziness and giddiness. That’s one of my favorite ICD codes.
July 16th, 2008 at 8:28 pm
What is an ICD code ? Yes.. I am stupid… but at least I am asking
I have coding that I have to useon our system of accident/incident reporting & I bet its not based on ICD based - even if I knew what it was.
You can reply to this to my e-mail if you want

Hey, Dr. Rob, try this one:
How about E919.8, “Struck by other specified machinery”, LOL!
You’d be surprised how common these accident codes really are, especially for cases involving Workman’s Comp. And where I am, they’ve started to require us to put in E codes for all types of accidents if we can determine them as such from the record. Most of the time the practitioners either don’t know they have to do so, or don’t know how to do so. Sometimes, despite the seemingly wide variety of stuff ICD-9-CM covers, you have to do a little stretching of codes to cover some of the freaky things that happen to people!
And for medstudentwife: ICD-9-CM codes are part of a worldwide classification for diseases and conditions, with modifications for use in reporting in the US. They’re used in medical records, for billing and reporting purposes, and in research as well. And even though their level of specificity is great, ICD-10, which the US hasn’t adopted yet (still in the research and development process) is even pickier, believe it or not!

“Accidental suffocation, due to being buried ICD-E913.3″
This one can cause nightmares!
July 16th, 2008 at 9:59 pm
LOL….I used to do ICD-9 coding for a Home Health Agency in the late 80’s and early 90’s, right at the beginning of the electronic record/electronic filing era. Talk about a headache. Is there an ICD-9 code for headache caused by filing ICD-9 codes?

Accident due to radioactive isotopes ICD-E926.5
This is my son’s favorite. I will let him explain ….
“What makes this code so amusing is that it is differentiated from accidents due to radiation…
The thing is…
All radiation is caused by radioactive isotopes. So…where’s the difference?”
Samuel Eagler
He just loved this! lol

There doesn’t seem to be a code for “broken hand caused by repeatedly punching f*ck-witted healthcare bureaucrat in face”.
July 17th, 2008 at 3:29 am
I thought The Worst-case Scenario Survival Handbook was real when it first came out (and I felt immensely thankful for it). So I found this coding list fascinating (in a disturbing sort of way). Perhaps it was the book’s inspiration…

Dizziness and giddiness was always my favorite too.
There was one I came across for injury for accidental submersion or drowning from a watercraft but not from injury TO the watercraft including walking the plank.
Good to know pirates are covered.

OK, my stupid question: What are NEC and NOS? I figured out abnd = abandoned, but can’t the those.
July 17th, 2008 at 1:52 pm
NEC = not elsewhere classified
NOS = not otherwise specified.
They are throw away terms meaning basically that you can’t be more specific.
July 17th, 2008 at 10:42 pm
The scary part is it all makes perfect sense to somebody.
July 18th, 2008 at 6:30 am
This is hilarious. What a beautiful example of bureaucracy at work. The humor, I suppose, was accidental.
July 18th, 2008 at 2:00 pm
What about a code for delirium produced by reading all these ICD codes.
July 18th, 2008 at 2:09 pm
the original source of ICD9 codes was the US military; as such, they needed to cover space-based warfare.

Hysterical Dr. Rob! Here’s my favorite: 799.3 debility, unspecified.

I decided to delurk to talk about my favorite ICD-9 and was beaten to it by the Happy Hospitalist.
Good ol’ 780.4 - Dizziness and Giddiness, which I suppose could be caused by being hit in the head with a keyboard.
July 22nd, 2008 at 12:10 pm
I’ll take dizziness and giddiness for $500, please…
